Spine Sports CTWelcome to our clinic! As a new patient coming in we will need some information from you.

If you wish, you can download these forms and fill them out before you come in to save yourself some wait time.

1.) Forms to be filled out if you are coming to our office(s) for chiropractic or physical therapy - download PDF form here

2.) Forms to be filled out if you are coming to our office(s) for an orthopedic surgical consult - download PDF form here

3.) Things to bring on your first visit to our practices:

  • Your Photo ID
  • Present and current Insurance Card and information
  • Auto Accident or Workers Compensation paperwork (if applicable)
  • Any previous diagnostic imagining (MRI, CT-SCAN, Xrays)
  • A referral (if needed) * most cases do not require a referral to our office(s)
  • Printed out paperwork from the links above and completed

Before you arrive: When you call the office to setup your appointment you will most likely speak to Jeanette or Suzanne. They will obtain some personal and insurance information from you as well as some general clinical information. They are not doctors, so they cannot give you any medical advice or recommendations. Once they have all your information, an appointment will be scheduled for you. Please allow at least 60 minutes for your initial examination and sometimes longer if x-rays are necessary.

During Your First Visit: On your initial visit to our office you will be greeted with a smile and given some paperwork to fill out about yourself, your current symptoms and past medical history.

You will then be placed in a private room and seen by a doctor to review the history of your condition. A detailed history is pivotal in order for us to better understand your condition, how it is affecting your daily activities and to accurately diagnose the cause of your symptoms.

The doctor will then perform a physical exam including orthopedic, neurological and functional exam. If the doctor deems necessary, x-rays will be taken on site.

Once the exam is complete, the doctor will render a tissue specific diagnosis and offer the appropriate treatment recommendations. Treatment is sometimes started on the first visit, but usually on the next office visit.

Before you leave, the doctor will answer all your questions and provide you with information and instruction such as modification of activities, exercises, proper biomechanics, Ice/Heat instructions, posture correction and ergonomic considerations.

The initial visit in a minimum of an hour and subsequent treatment visits range from 30-60 minutes.
